About the Role
The Coordinator Home Support is responsible for leading the delivery of high quality home support services as part of the Supported and Connected Living (Aged and Disability) Department.

You will work collaboratively with other leaders to ensure individuals and their families/carers receive high quality services within the guidelines of the Commonwealth Home Support Programme (CHSP) and the Home and Community Care Program for Young People (HACC PYP).

  • Coordinate high quality, efficient, accessible, accountable and appropriate supports, and services
  • Foster positive working relationships and partnerships operationally and strategically internally and across the community
  • Build workforce capacity, diversity and business flexibility to adapt to changes in the sector
  • Develop and implement innovative program and services as part of Age Friendly Darebin strategy
  • Drive and support a culture of safe work practices, responsive customer service and inclusive continuous quality improvement

Skills & Experience
  • A tertiary qualification in health, welfare or related area
  • Substantial management experience leading large home support services
  • Demonstrated experience developing and implementing new services
  • Demonstrated experience of quality improvement and standards
  • Experience developing values-based culture
